Starting September 8th, we’re temporarily bringing our Danielson and Putnam clinics together under one roof in Putnam. Your care, your clinicians, and your treatment plan stay the same — the only thing changing is the address.

We’re also welcoming David Hoyle, PT, DPT, MA, OCS, MTC, COHP, CWC, CEAS, an experienced local clinician, to the Putnam team. As always, no referral is needed to get started. The therapists are knowledgeable, professional, and compassionate. The entire staff is friendly, supportive and the atmosphere extremely positive which is such a plus when you're on a difficult road of recovery. I've been to other facilities and would not do my PT anywhere else.

I am extremely happy with the results of my treatment at PTSMC! Kyleen listened carefully, was quick to grasp my situation, and was very thorough in recommending apparently the perfect exercises, judging from my mobility and complete lack of pain now. The tone of the office is professional, pleasant, and relaxed. Five stars for PTSMC!

My husband got knee surgery last December and he went to PT at PTSMC Putnam for the last 5 months. The staff was always friendly and courteous. They understood his goals and took those into consideration while helping him progress forward. Ashley was great! My husband definitely recommends them!

Physical therapy & sport Medicine Centers is the best place to go for your physical therapy needs .. they are professional, they treat you very well.. they asked the right questions. The Physical therapy crew are excellent, funny , it makes it more fun when you are dealing with pain. Everyone that works there has a smile. Which it makes me try harder.

I would 110% recommend this facility to many people. The facility has great staff. They make you feel comfortable. Everyone is extremely friendly and know what they’re doing. The first time I went to this facility was two years ago for a shoulder impingement I then went back for a calf injury. Both injuries were figured out and fixed within 3 to 4 months of consistent appointments. What an amazing team. Thank you

I've been getting alot of physical therapy from these amazing people. Before coming to them I wasn't able to do alot of things do to a injury since I've been going there for physical therapy my leg has gotten alot stronger and I can do more things. The staff really works excellent with you. If anyone ever needs physical therapy I highly recommend this place.

Just finished a round of PT with Tom. He is really on top of things: Well informed accompanied by a broad perspective. He was able to break down my MFI report and communicates the news better than my doctor (great metaphors for what is actually going on with my knee).
